Anterior Aesthetics: Tissue Training, Immediate-Loading vs. Delayed Protocols, and Restorative Material Choices

Aesthetic dentistry is a rapidly changing discipline. As more anterior teeth are being replaced with dental implants, factors such as tissue management, loading, and materials need to be considered. Proper diagnosis and treatment planning is essential for success.

At the conclusion of this program, each participant should be able to

  • Recognize the diagnostic needs for anterior dental implant therapy
  • Understand the biologic principles of implant mapping and decision making
  • Understand which provisional restorations are most appropriate for a given restorative plan

Session 1 Overview

Map

  • Implant position (M/D/ & B/L & countersink)

Bone levels

  • Adjacent teeth
  • Adjacent implant(s) (M/D & Buccal)

Soft tissue volume

  • Thin or thick biotype

Symmetry

  • Contralateral teeth and gingiva

Tooth

  • Proportions & contours
  • Shape

Spatial Rules

  • 1.5mm between implant/tooth
  • 2mm bone at buccal
  • 3mm implant/implant
  • 5mm or less from crest of bone to interproximal contact of restoration and adjacent tooth for at least 60% of interdental papillae/embrasure fill

Questions to help determine if there really is a problem

  • Is the implant too far buccal or too far palatal?
  • Is there too much soft tissue or not enough soft tissue?
  • Is there enough space (1.5 mm) between the implant and adjacent tooth?

  • Use wax-up to determine symmetry, proportions, gingival levels
  • The bone sets the tone for soft tissue levels
  • Soft tissue always follows the bone and something else
  • Delayed provisionalization can be appropriate depending on the case
